What is AMH?

AMH stands for Anti-Mullerian Hormone.

This is a female hormone produced by the primordial cells or sleeping follicles in a woman’s ovary.

The higher number of follicles you have, the higher will be your AMH level. In short, the AMH level indicates a woman’s ovarian reserve or egg reserve.

It’s important to note here that every girl child is born with a specific number of eggs. Once these eggs run out, a woman’s body cannot produce another one.

At the time of birth, the number of these follicles is in the millions.

But when the girl reaches puberty, it reduces just to some lacs. Gradually these numbers keep on decreasing.

Once all eggs are destroyed and the ovary runs out of eggs, a woman achieves Menopause.

So where does this leave us with AMH?

Well, if a woman has a low AMH level, there is very little we can do to increase it.

We (IVF specialists) try to pick the best eggs from the remaining ones for further procedures.

So what you can do as a patient is to act fast before the egg reserve runs out completely.

Can you conceive naturally with low AMH levels?

Of course, you can.

There are a lot of women who conceive naturally, even if they have low AMH levels. But there are a lot of factors at play here.

E.g.
Age matters the most. You can try naturally and conceive if you are 25 to 26 years old.


But remember,

The exact value of AMH is also important. 1.5 ng/ml AMH is considered low, but a woman with this value has more probability of conceiving naturally as compared to a woman with an AMH level of 0.5 ng/ml.

In both cases, you should try aggressively to conceive in the next 2 to 3 months.

But if you are unable to do so, it’s better to consult an IVF specialist. The reason is the more time you waste, the more your eggs are destroyed.

So as the time elapses, the possibility of an IVF procedure using your own egg reduces too. So make sure you act fast.

Your clock is already ticking, and hence you have to be fast.

What other steps can you take?

Apart from trying aggressively, you can take vitamin D supplements.

These supplements are known to show some positive effects in low AMH cases.

Sometimes we also offer Testosterone Gel and DHEA to the patients. But remember, these steps are useful only when your Age is young.

If your Age is more than 35 years, early IVF is the best choice you can make.

The reason is that at this Age, the number of eggs is low, and their quality also starts becoming poor naturally due to chromosomal abnormalities.

 What to do if your AMH levels are low?

You can do a couple of things if you are diagnosed with low AMH levels.

For e.g.
You can take vitamin D supplements and use DHEA or testosterone gels to improve the quality of the eggs after consultation with the fertility specialist.


Moreover, you should improve your lifestyle to ensure the good quality of eggs.

Regularly exercise, adopt a healthy diet like fresh fruits and veggies and increase protein intake.

Add nuts and dry fruits to your diet, along with a reduction in saturated and fat-laden fruits. Lose some weight and get fitter.

All these steps can have a positive impact on your condition. But the most important thing to do is Stop Wasting your Time.

The more time you wait to make the important decision to consult an IVF specialist, the less healthy eggs you are left with.

Stimulation Protocols for egg collect

Certain techniques might help you with the stimulation. This includes:

  •   Antagonist Protocol

Under this protocol, we give injections to patients from the 2nd day of the month. After the 9th or 10th day, we retrieve the eggs to develop multiple eggs.

We aim to collect as many good eggs as possible to increase the possibility of a successful IVF.

  • Duostim Protocol

Although we aim for 15 to 20 eggs, it’s difficult for patients with low AMH levels.

It’s possible to retrieve only 3 to 4 eggs. In such cases, we adopt the Duostim Protocol.

Under this procedure, we again inject the patient with the stimulant on the 5th day after the first egg reterival,
i.e. Luteal Phase Stimulation Protocol.

This saves a lot of time and helps us collect more eggs. So we can create more embryos, increasing the possibility of a successful pregnancy.

Ovarian Rejuvenation

This is the latest and most advanced procedure for patients with low AMH values.

In the procedure, we insert Platelet-Rich Plasma (PRP) in your ovaries by Ultrasound or Laparoscopy.

Once done, we try to stimulate them to produce more eggs.

Although the results of Ovarian Rejuvenation are promising in Younger Ages, the technology is still catching up.

But we are using it on some of our patients, offering some good results.

Remember, Age is the most important factor affecting AMH levels, no matter your chosen treatment.

When you do not have follicles in the ovary, no treatment or procedure can help you generate eggs. 

And if nothing works and no treatment is delivering any results, there is always an option for going ahead with the donor’s egg.

Wrapping Up

Low AMH is generally an asymptomatic condition, so until and unless you don’t get the test, you won’t even notice it.

Apart from that, some subtle signs mentioned above might give an indication to you.

So if you are experiencing such signs, it’s better to consult your IVF specialist or doctor immediately.
