Zero sperm motility treatment explained

What is zero sperm motility?

“Zero sperm motility is a male infertility condition that is also known as 100% immotile sperm. It means that all the sperm present in semen are not moving – i.e., they do not swim. This condition is referred to as asthenozoospermia. In cases where none of the sperm are mobile, the condition is known as complete asthenozoospermia,” mentioned Dr. Gunjan Gupta, well-known gynaecologist Ghaziabad and Medical Director & Founder of Gunjan IVF World – the third time winner of North India’s best IVF clinic chain by Economic Times. The treatment of zero motility depends on the underlying cause of the condition,” she added.

Types of zero motility

There are mainly two types of zero motility

  • Dead sperm (necrospermia): A condition where all sperm are non-viable or dead.
  • Live but immotile sperm: A condition where sperm are alive but they cannot move due to defects in structure and function

How to diagnose the condition?

“Zero sperm motility can be diagnosed through a semen analysis. It might take two or more assessments to diagnose the condition. A fertility specialist would also use a vital staining method to check if sperm are alive but immotile. Other methods of diagnosis may include hormone tests, genetic testing, scrotal ultrasound and possible testicular biopsy too,” stated Dr. Garima Sharma from Gunjan IVF World, Janakpuri.

Treatment options available

Intracytoplasmic Sperm Injection (ICSI): “In cases where sperm is alive but immotile, ICSI treatment approach is adopted by a fertility specialist. In this single viable sperm is injected directly into the egg. In this motility is not required if the sperm is alive,” said Dr. Gunjan Gupta.

Testicular Sperm extraction (TESE): In cases where there is no alive sperm in the ejaculate, sperm is retrieved from the testicles directly. Live sperm can be found in the testicles when none can be found in the ejaculate. This extracted sperm is then used for ICSI.

Genetic Counselling: In cases where the cause of zero sperm motility is related to genetic anomalies, then genetic counselling is necessary.

Donor sperm can also be considered when no sperm is found.

Supportive steps to improve zero motility

  • Lifestyle modification: By avoiding heat, smoking, alcohol consumption, and toxins.
  • Antioxidants: Consuming antioxidants can improve sperm quality in borderline cases.
  • Treatment of infection: In cases where infection is affecting sperm function, this is absolutely necessary
