Low AMH, Age 35 yrs

They told me my AMH was low at this age, I thought motherhood was slipping away.

She came to us deeply worried after being told that her AMH was very low and that her chances of having a baby were limited. After years of trying and unsuccessful ovulation induction, she felt she had almost run out of options. Her scans showed a very low ovarian reserve, with only a couple of antral follicles.

Instead of focusing only on the AMH number, we focused on what her ovaries could still produce. Careful stimulation resulted in two mature follicles. One egg was retrieved, fertilised through ICSI and developed into a good-quality embryo. A fresh embryo transfer was performed. Her pregnancy test turned positive, turning a journey filled with fear into one filled with hope.

Severe male factor

There was no sperm in the report, we thought our dream was over.

The couple came to us after a semen analysis showed no sperm. They had already been advised hormonal medicines elsewhere, but there was no improvement. Further evaluation suggested obstructive azoospermia, where sperm production may be preserved but sperm cannot reach the ejaculate. The couple was understandably anxious about whether biological parenthood was still possible.

We explained that the absence of sperm in the semen does not always mean the absence of sperm production. After appropriate evaluation, we planned surgical sperm retrieval. PESA successfully retrieved sperm, which was used for ICSI with the partner's eggs. The embryo transfer resulted in a positive pregnancy, and their journey ultimately led to the birth of a healthy baby boy

Tubal factor infertility

We thought IVF was our only option. Then we realised tubes were the problem.

After several years of trying to conceive, the couple came to us following unsuccessful fertility treatment elsewhere. Evaluation revealed a hydrosalpinx, with fluid-filled, damaged fallopian tube affecting the chances of implantation. We explained that simply proceeding with IVF without addressing the tubal problem could compromise the outcome.

We planned treatment in a stepwise manner, addressing the tubal pathology before embryo transfer. After appropriate management and allowing time for recovery, the couple proceeded with IVF. A good-quality embryo was transferred, and the pregnancy test was positive. What initially felt like another failed-treatment story became a successful pregnancy after identifying and treating the underlying cause.
